Human Exposure Report 

​Report a new event that involves a potential rabies exposure to a human.  A potential exposure includes, but is not limited to, a bite or scratch from a mammal that breaks the skin.

By law, any person having knowledge of any animal which has potentially exposed a person to rabies should immediately notify their local Health Department.  S
ubmitting a completed form meets that requirement.
